Thrive Membership

Support and strengthen your health with a plan designed for those managing two or more chronic conditions. This membership offers more frequent check-ins, personalized guidance, and coordinated care to help you stay on track and see real progress.
Medicare Membership

Traditional Medicare only (we do not accept Medicare Advantage). This membership covers services Medicare does not, providing extended visits, personalized lifestyle guidance, and whole-person wellness support.
Student Membership

For students ages 13 through college. Parent or guardian consent required for minors under 18.

A 12-week, proactive approach to heart health that includes a full lab panel, InBody body composition scan, and coronary risk assessment. The program features three visits — an initial visit, a six-week follow-up, and a 12-week final visit — each focused on optimizing cholesterol, blood pressure, and overall cardiovascular wellness through lifestyle change and medical oversight.
Diabetes Prevention & Management

A 12-week program designed to prevent or reverse prediabetes and manage diabetes naturally. Includes fasting labs, an InBody body composition scan, nutrition and activity tracking, and personalized lifestyle coaching. Participants meet at the start, six weeks, and 12 weeks to monitor progress, improve insulin sensitivity, and build sustainable habits.
